Letter format of pan surrender

Dear Experts,
there is one canadian citizen person of indian origin, actually her parents are Indian, but she she born in Canada so she having a canada citizenship. So few months back she has applied for PAN card on the basis of her Indian ID and Address proofs now her citizen ship is updated as Indian in Income Tax Department because she has applied for PAN in form 49A rather than 49AA??? right
Can anyone having a format of letter for surrendering such PAN to Assessing officer.
