How tds refund could be claimed

01 August 2016 No pan was submitted in the bank by me and tds deducted @ 20% and they have provided me a certificate in which there is no such number as "tds certificate number". So how can i claim the refund of tds as there is no such thing available in the documents provided by the bank.
