How tds refund could be claimed

No pan was submitted in the bank by me and tds deducted @ 20% and they have provided me a certificate in which there is no such number as "tds certificate number". So how can i claim the refund of tds as there is no such thing available in the documents provided by the bank.

Contact your jurisdictional Assessing Officer of income tax. ITO will allow the claim against manual/physical return
