my friend have to make Pan card and required a certificate from guzzeted officer. Would you please know me the format for the said certificate. regards Dharmendra Jain

This is to certify that the photograph affixed along side is the true resemblence of Mr. Dharmendra Jain ,aged about 40 years,with date of birth as 14-10-1971, son of Mr.Narender Jain, who is residing at 11/423, Jain house, CA club india road, Andheri, Mumbai-400001. This certificate is issued upon his request to apply for PAN card as per the Income tax Act,1961.
PS:-1) This is to be obtained in the letter head of the G officer. 2) Applicants photo has to be affixed in the certificate and the G officer should sign accross the photo.
