Form no. inc – 10 verification of signature of subscribers

1. While attesting the photograph, do attesting authority has to sign and stamp on the photograph, if stamp need to be given on photograph than what type of stamp need to be used?
2. Where to mention the address of attesting authority?
3. Are attesting authority and witness are same person, if not than what type of details of witness need to be given?
Please anyone answer my above doubts.
