04 September 2013 Dear Experts,

I filed Form DIN 1 and the Approved DIN was alloted to the applicant. But, while filling the Middle Name and Last Name of Applicant and Middle Name and Last Name of Applicants Father's Name got Interchanged. As a result,the Applicant's Middle name and Last Name are appearing as interchanged in the DIN Database.

One of the possible solution for fixation of this issue is by way of Filing Form DIN 4 and correcting the details therein.

Please Advise the Correct procedure for fixation of this issue.

04 September 2013 Please check what details you have given in DIN 1 or any affidavit attached there of with regards to your name with father name.

If any correction required then file DIN 4 with MCA.

04 September 2013 If the name approved matched with the name as in PAN card, then DIN -4 will not serve the purpose unless changes were made in the PAN data base.

04 September 2013 The error is elaborated as under :-

For Instance:-
Name of applicant is RAM SHYAM MOHAN

and in the Form DIN I it is filed by mistake as RAM MOHAN SHYAM i.e. LAST NAME in place of MIDDLE NAME and MIDDLE NAME in place of Last Name.

The details in PAN Card and Afiidavit are correct. Please advise accordingly.
