You received Form 112 because you would have done any other course along with CA without the permission of ICAI.
Send following to Regional Office of ICAI:
1) Form 112 duly filled in and signed.
2) Condonation Fees.
3) Reason of not getting permission from ICAI.
4) Your employer's comments on his Letterhead.
