Please let me know what are the Proof of Documents required to be procured from Doctor for availing the deduction of Preventive Health Check-up u/s 80D.
Is there any particular format in this regard.

15 February 2013 Reports of health check up from a well known Health Center or Hospital along with the bill will be sufficient .

No Particular Format has been prescribed so far.

Plz confirm the following:
1. If the document can be obtained by a Doctor on its own letter-head instead of a Hospital document;
2. Moreover, should the matter be printed on Report or could it be hand-written also; and
3. Would a cash receipt do good in this regard.

15 February 2013 Only Good Point you have mentioned is Cash Receipt will do. Yes... it will do - limit Rs. 5000/-.

Other points mentioned by you may spoil the health of the assessee's case in IT.
