27 August 2015 There is only one way out, to submit application with fee along with copies of PAN card and address proof attested by bank manger/gazetted officer/ postmaster. The application form to be filled in blue ink only and the attestor should affix seal, name and mobile number. After the recent supreme court verdict, if aadhaar card is used as address proof, certifying authority will approve application only after 'call verification' through the applicants mobile number given in the application form.
