12 October 2011 Applicants name as per PAN CARD - RAMESH KUMAR K applicants father name as per PAN CARD KARUNAKARAN G. What is the first name, last name and middle name for both for DIN application purpose. Name as mentioned in the Passport for applicant and his father as RAMESH - KARUNAKARAN for both respectively. How to apply the DIN form without getting rejected. Whether PAN ID proof is mandatory for applying DIN?

12 October 2011 Presuming that the applicant's name as in PAN data base is RAMESH KUMAR K , try for DIN-1 as follows:-

Applicant's name
1st name - RAMESH
middle name - KUMAR
last name - K

Father's name
1st name -KARUNAKARAN
middle name- leave blank
last name- G

Attested copy of PAN is a mandatory attachment.
If pass port is the address proof, affidavit is also necessary since name in PAN and passport is not matching.
