16 June 2015 1. Copy of PAN card and address proof (preferably passport, aadhar card) attested by Gazetted officer/ bank manager/ post master.

2. Passport size photograph.


3.Application form. Can be obtained from 'certifying authority'. To be filled in and signed by blue ink only. One such authorities reference is available in the following link.

4. To pay the Fees in advance.

5. DSC will be issued with in a couple of days in USB TOKEN only.
