Kindly provide format of CA Certificate for FCGPR reporting to RBI for bonus shares? And as per which RBI notification valuation for the same is not required?

There is no valuation format prescribed. You may refer to master circular on fdi issued by Rbi on 1.7.2011. Let me know in case of further issues.
