Articleship completion problem

My friend's articleship was completed in Last year dec 2015.But he didn't sent from 108 to institute & now it has elapsed about 6 month & he needs to send in a letter of condolation as well as a letter stating reason for the same.

So the problem is what reason to be sent to institute .Pls experts reply & help ASAP.

Re. Condonation of Delay in Submission of form 108
