22 September 2015 Sir/Madam,

I need to submit form 109 as I am going to terminate my existing article training, hence I've completed just 4 months. What is the procedure to submit form 109? and how many of copies should I submit? How can I fill my practical experience in a terms of week? Please let me advice about my queries as soon as possible, your answers are hearty welcome.

22 September 2015 4 months means 16 weeks. Accordingly you can fill the form 109.

make 3 copies, one for you, one for your principal and one for ICAI.

Get the form 109, fill it and get it signed by your Principal.
