As per FA 2012 new deduction allowed for preventive health check up of Rs.5000/-

In this regard i want to know follwing

1) whether Rs.5000/- allowed in addition to Rs.15000/- or Rs.20000/- or it is included in Rs.15000/- or Rs.20000/- bracket
2) Whether payment can be made in cash?
3) whether any the check up should be in any govt or specified hospital?
4)whether the deduction is assessee wise?

1. Rs. 5000/- included in Rs. 15000/-

2. Payment can be made from any mode for preventive Check-up but for other payment must be made other than cash.

3. Check up can be from any hospital
4. Deduction is only for Individual
