Dear All,
C Forms (Original & Duplicate)issued by our customer were lost in transit. So you are requested to advise me the procedures if any, to solve my problem.

Apply for DUPLICATE Form to assessing officer.Follow due procedure under CST Act in case of lost declarations.Contact to your assessing officer.
