Do we need sign of chartered accountant on form no.2 for applying mebership no.
2.If i didnt get attested on PE-II certificate is it ok.

You have to get attested by CA or any gazetted officer, its must...

form no.2 need not be sign , but photo copy of documents are to be attested by CA.

I have applied for membership and I have not attested any CA / Gazetted Officer signature on all the photocopies of certificates submitted along with form 2. It is not required because u urself is a CA. If it is already attested its ok.
U can send the photocopies of all certificates without attestation also. No need of stamps and dont worry if any one of the documents is not attested too.

You need attestation by somebody else. Self attestation in what capacity ??? (as you r applying for membership no.). however, seal of the member attesting is not required.
