Transfer of articleship in 2nd year

Sir/Madam,
My hometown is away from where i am doing my articleship but due to continous bad health, and skin allergies and infections, doctors have suggested me to be at home and continue my job/training and have given it in written. I work in kolkata branch office since 1.5 years (but my registration is from bombay due to my principal,s address is there). Please guide me how to proceed for transfer to hometown ca firm

U must have to obtain a Medical Certificate from a government Hospital, which supports ur illness in Transfer. And of course Principal's Consent is must.

Sir i have such medical certificate obtained from hometown indicating homely requirements and prescriptions of doctor i have been treated from.

But sir institute site shows that i need to firstly send only application for transfer and later form 109 after receiving approval from icai. is it right pls help???
