28 September 2013 u need govt doctors certificate stating the same and should be attested and all other medical bills and reports main one you should attach in the form of copy and retain the original one.

28 September 2013 My advice was under the assumption that you have completed your first year of articleship and if not the directly fill form 109 without ant attachment in case if its first year of your articleship not completed.
