I joined ca in direct entry. I complete my articleship of 5 months. Now i don't want to do CA. What is procedure of termination?

I fell up form 109, now icai require affidavite in Rs. 100 Stamp and NOC letter of Principle. can you send me Affidavite format & NOC format?

No,plain form 109 duly signed by your boss is enough
