Is it compulsory to deduct TDS from salary every month. If not deducted what is the penalty and what are the deadlines to pay TDS deducted from the salary.

No. It is not compulsory to deduct TDS every month on salary.
What you need to do deduct the entire tax payable over the period.

Clause 3 of CBDT Circular No. 8/2010 [F.No. 275/192/2009-IT(B)], dated 13-12-2010 SAYS INCOME TAX HAS TO BE DEDUCTED FROM SALARY EVERY MONTH AT THE TIME OF PAYMENT.

Mr. Warrier is right. Calcualte the anuual liability, divide by 12 and accordingly deduct tax every month.
