I want to add services in client ST2,other than amended ST 1 which proof is required,LOA is required if yes,is there any specific format for LOA

No specific format. Just write a letter to your jurisdictional officer requesting him to amend the registration certificate whilst informing the details about the new category of services provided and the date of commencement of such services, enclosing the amended ST-1 generated on-line.
