Which form is used for service tax...can I.have a link where I can see it....and how it is paid through a bank or else?

1) Service tax is to be paid in GAR-7 form which is available in http://www.cbec.gov.in/st_front_jan09.pdf
2) E-payment is mandatory in certain cases and those are covered in notification 1/2010 dated 19/2/2010.
