PLEASE PROVIDE THE FORMAT OF ANNUAL REPORT IF THE SHARES ARE TRANSFERRED DURING THE CURRENT REPORTING PERIOD AT TWO DIFFERENT DATES BY THE SAME SHAREHOLDER

In that case you do need to have format of Annual Report.
You just take registration of these transfer in two different dated.
The details of transfer will be mentioned in Annual Return in which you will mentioned the transfer details giveing details of both dates and submit the same with Form-20B annually with in 60 days from the date of AGM with ROC.

Transaction related to share transfer is required to report to ROC on annual basis. The same is required to be disclosed in Annual Return to be filed with ROC annually through filing of form 20B.
