Revalidation query...plz help!!

hello...
i have to apply for revalidation...my query is how much do i have to pay..some say itz 500, whereas in the ca magazine that comes every month, itz mentioned 400 for ipcc revalidation...plz clarify..
(i will be downloading the form online)
another thing is that where should i submit the form?...mumbai branch or the icai branch in my hometown??
Also..how will the inst intimate me of whether my revalidation has been accepted or not..??

it is Rs 500 per paper. it is advisable to file online. if you submit physically, you can submit it to
