and how do i show medical store in hospital in GST?

Hospital is exempt from GST take GST registration for medical stores.

if the hospital and medical store are covered in same PAN, then should the hospital receipts should be shown in exempt supplies in GSTR-3B and GSTR-1 ?

YES, YOU HAVE TO SHOW HOSPITAL TURNOVER AS EXEMPT SUPPLY AND MEDICINE SUPPLY AS TAXABLE SUPPLY
