I ISSUED H FORM TO MY SUPPLIER, IN HIS OFFICE HE HAS LOST THE H FORM, NOW HE IS ASKING DUPLICATE H FORM. CAN U TELL ME THE PROCEDURE TO GET THE DUPLICATE H FORM.

you can apply for "Duplicate" form "H". in addition to regular documents following documents are manadetory like copy of F.I.R., 2. Notice about this lost in national / local news papers (English & Local Lenguage)and 3. an indemnity bond of maximum value of tax involved in that transaction.
