Prescribed format for TDS on Transport Contractor for PAN Qu

Sir,
I wanna know that how can I get a format to be filed with Income Tax Deptt. for no TDS deducted from Transport Contractor, being furnishing his PAN Card.
Also, what is the last day to file that format.
W.e.f 1st, October 2009 for TDS rulings

hello,
any person to whom the any amount is paid under section 194 c,where no tax is to be deducted or t be deducted with lower rate than he has to apply assessing officer in form no 13 to obtain the certificate as per the provision of section 197 read with rule 28 and 28AA

Hello Mr. Nischint, Isn't this is form required in the case...........
Form No 15-I, the one which a truck holder has to submit with the transport company under which he is plying his trucks and
Form No 15-J, the one which a transport company has to submit with Income Tax Department.
These situations are letting me puzzled, where to go...............
