HI all, Can anyone please provide me the format of " declaration for not deducting PF" to be collected from the employees ( Employees joined with a basic salary of 12200 p.m. and not opting for PF.)( no previous employment)

I am herewith forwarding the declaration for your kind perusal
“I wish to inform you that I was not a member of EPF Scheme anywhere before joining this establishment. As my basic+DA wages is above statutory wage limit of Rs.6500/- for computing P F, I am not inclined to become EPF member at present. I am aware of the consequences in giving this declaration.”
This can be modified according to the company policy.
