If There is Change in The name of Pan Card Holder
& There is Proof Of Certified by Gazetted Officer then What is the Proof Required of Gazetted Officer.

Proof of Gazetted Officer is not required. I think, there is some confusion between Gazetted Officer and Gazette. Sometimes for change in name -publication of the name in Official Gazette of State Govt. is required.
