Please refer notification 10(E)dt 6/1/1994 reported in 205 ITR (St) 121 and notification 1302 dt 28/12/1999 reported in 248 ITR (St) 258.

The second cited notification is not applicable for KERALA state. The first cited notification is not visible in www.incometaxindia.gov.in. Therefore,you may refer the ITR referred by Mr. KantiJain.
