Not showing full details of exam form in pdf

dear sir,
i have enrolled myself for nov 2011 exams through online form. payment transaction is also successful but the pdf form is not showing the full information filled in it(some column are showing blank),may be information is not saved but i have filled all detail. what should i do now? please make reply for this as soon as possible.

Do one thing you should write a letter to the Dy. Director Examination stating all facts.
