normalising a dormat comapny

dear sir,
a client of us is having a company the status of which shows it as a dormant as they have not filled any form since 2006. now i want to make it active. i come to know that for 61 is required to be filled for it.
i want to know the detailed prcedure regarding the filling of form and attachments to be given. i also want to know that whether the balance sheets and annual returns of all the years are to be filled and if yes whether it is to be filled as soon as the form 61 us filled.
your valuable suggetions are expected. kindly also send copy of attachments to my e-mail as early as possible.

1) e-form 61 has to be filed. Attachment to the said forms are an application supported by an affidavit executed in Rs. 100/- non judicial stamp paper. affidavit should be notarized.
2)Roc will approve the application and give 21 days time to update all the pending anuual / compliance related filing.
3) Upon the compliance stated above and the said forms were approved by MCA , the status of the company will be changed to 'active' which can be seen through 'company master data' in MCA Portal.

Kindly send me a copy of the said appication and affidavit. I also want to know that is the EES 2011 still in operation or if it is withdrawn how a compny can be closed now. Is it also required to normalise a dormant company before closure procedure is taken.

Fast track exit scheme for defunct companies is introduced vide general circular 36/2011 dated 7/6/2011 w.e.f 3/7/2011.
