New pan application for huf

My friend wants to create HUF.
He wants to apply for HUF PAN No.

Pls tell me that is there any requirement to furnish a affidavit made by karta stating name of fathers and his coparcners.

Pls provide me the format for affidevit so that we can help him in this matter.

30 January 2013 just apply the pan card in form 49a., and mark as huf ., and keep the proof of the address and id., no need for any affidavit
