My original h form is lost and my case is in appeal

11 December 2014 Dear sir we are supplier and we have recd h form from purchase on time f.y 08-09. The officer has pass exparte order agst h form as it was shown in audit report pending status..now the case is with jt commissioner in appeal.i have xerox copy of h form. How to justify him.and how to convience him with xxerox copy. Pls give suggesdtion very urgent

12 December 2014 If the selling dealer has lost the 'H' form,he has to send a letter to the purchasing dealer to provide a duplicate copy og the same along with,xerox copy of the lost form,if any,and an 'Indemnity Bond' as given in Form G.The concerned authority shall issue a duplicate 'H'form,clearly marking on all the three i.e original,duplicate and triplicate,in red ink as duplicate copy of the earlier form.On producing this duplicate 'H'form the matter will be settled. In the meanwhile a request may be made to the appellate authority to give some extension of time.......mjk
