Modification in articleship completion date

I take a transfer in my first year of training but at the time of re-registration i put the re-registration date as 18th Nov. in place of 15th November 2014 wrongly since got the form for re-registration at 18th Nov. 2014, i didn't know the fact of this mistake.
But now i am facing a problem with my examination attempt .
Can any body help me how can apply CA institute for change the re-registration and Completion date in my case and what type of proof i need to send along with the application ???

It is difficult so to do NOW.
You may try to do so by writing a request letter to ICAI, along with the reasons as to how the mistake was done by you as well as your Principal, in putting the date as 18th in place of 15th.... (The job is difficult)
