Lost my itt certificate

09 February 2013 Hello Sir,
I am going to fill the exam form for may 2013, but I lost my ITT and Orientation Certificate.
In the exam form they ask the number of ITT certificate, Kindly suggest me how can I fill the form without ITT number? & also tell me what is the procedure to issue duplicate certificate?

Kindly approach the concerned Regional office of ICAI. Normally, they would ask you to submit an application on a plain paper for issue of duplicate certificate. Before going to the regional office, make a call to the helpline numbers available on ICAI and regional office websites to save time.
