whether tds deducted details should be included in IT Sheet of ITR 4 like bsr code, date of credit into govt a/c, sr.no. etc.???????

No. TDS deducted details should be included in TDS sheet. In IT sheet only details of Advance Tax, Self Assessment Tax is required to be given.
