for PF you need to approach the Regional PF office in your city where you stay (refer PF website to know the area.
For ESI also you need to approach the nearest ESI center were in your jurisdiction falls.
If your professional services exceeds 10lakhs then, you need to pay service tax. So, you need to even get registered for Service Tax - ST1
For PT, you can down load the PT registration forms frow the website, you can check the website of Gurjarath PT offices and get it registered.
