format of indemnity bond for it refund.....

23 July 2010 Hi ,

I spoke to the Income tax officer for the IT refund. He said the status in the database does not matches with my claim and asked me to submit the Indemnity bond on Rs100 stamp paper

It would be great if anyone send me the format of Indemnity bond for the IT refund.

Dear Sir,

I, XYZ, Son of Shri ABC, having residence at PQR, Mumbai – 400 001 do hereby affirm & confirm that :

1) I have filed my return of income for A.Y. 2004-05, 2006-07, 2007-08 and thereby claimed refund from Income Tax department. The details of the said A.Y. are given below:
