06 November 2013 There is no prescribed format for confirmation. it should include following paragraph in your letter:-
1. Title of the letter :- Confirmation of balance
2.information relating account balance i.e you can attach account copy
3. statement of acknowledgement and request for sending back a copy of acknowledgement
