Can someone please provide a format of Affidavit to be made by a Chartered Accountant to ROC to submit documents on behalf of Client?

A & R Associates, Chartered Accountants, A & R Buildings, …………Road, Cochin-682016 to present the Memorandum and Articles of Association of the Company to the Registrar of Companies , CRC, Haryana-122050, to make corrections, alterations, additions and deletions, if any, in the said and supporting documents and to make statements on our behalf and
We, the undersigned do hereby agree to ratify and confirm all acts done by the above Chartered Accountant in the said matter as our own acts, as if done by us to all intents and purposes.
