Form H is issued by the receiver Branch to the Giver Branch.This will enable the sender to claim tax exemption on the ground that the goods have been transferred to their own Branches.
Form D is given by Govt. agencies when they are purchasing without tax.

Form H is issued by the exporter to the dealer making a penultimate sale for export.
Form E. Pl see the answer(1st para) given by Mr. Srinivasulu.
Form D is omitted w.e.f 01/04/2007. Before that date it was issued by Government during interstate purchase.
