Query in filling the Form DIN-4.

If the applicant wishes to update only the permanent & present address, can we fill only the following fields in Form DIN-4 :

Field No.1 and Field No.2, thereafter
Field No.11 and Field No.12 and
Page 2 (leaving the applicant's name and his father's name blank)

Experts' may leave their expert views.
