Forget to take printout of surrender form of service tax

I have a problam and problam is that I forget to take printout of surrender form of service tax registration in aces web site and after that it is required to be submit this form in service tax department please tell me how i can take this printout

02 January 2012 No need to worry. Write to customer care of aces at cheenai, you may also call them on toll free number. ask them for transaction no or soft copy of your surreder request for ST registration.

write a letter to jurisdictional Service Tax authority with whom you were registered informing surrender of ST registration certificate. make sure to file all your returuns and obtain on line e acknolwledgment for the same for records purpsoe. You have to retained all service tax returns, documents for cenvat credit if any for audit purpsoe till five years.
