Filling up form 107

14 April 2015 I have completed my articleship on 27/02/2015. I recieved a letter from icai saying i have taken excess leave of 4 days for which form 107 is to be submitted. I have taken the printout of the same to fill it up. The form seems to be confusing to fill. It would be helpful for me if someone sends me a sample of filled form 107 for refrence to my email id brameash@gmail.com. It would be really helpful if someone responds to the message at the earliest.
