Error in check issue (refund)

03 December 2011 i made an Income tax return by making assessee name wrongly and i revised the return correctly and receipt for IT acknowledgement has been came for both original and revised return.

Now the IT dept has issued the Refund Check in the name (Originally filed) and the postman has returned the check due to false name...

so, how can i get the check from IT department. provide me the procedure....

03 December 2011 upload e rectification u/s 154, download from incometaxindiaefilling.gov.in . this form include all rectification column which mistake had made in original or revised return.
