Fathers name - balayya
Grand fathers name - chinnappa

Which of the following is correct ?

1. Iam BHASKAR B and my fathers name balayya appearing is single name.............

or
2.Iam BHASKAR BALAYYA and my fathers name balayya appearing..........

how it is to be signed? BHASKAR BALAYYA OR BHASKAR B ?

01 January 2010 Affidavit should start with the 1st option suggested by u ie BHASKAR B.
So also the deponent's name at the end of the affidavit,just below the signature, should be BHASKAR B.
