1 Identity proof : Ration card, Voter Id or passport.
2 Passport size photographs of proprietor : 4 nos
3 If busines premises is own :Property title deed, property tax challan.
4 If busines premises is rental : copy of rental agreement, electricity bill for 2 months
5 Introduction letter from 2 registered dealer having same Sales tax jurisdiction.
6 Fixed Deposit for Rs.500/- in favour of : CTO____ circle account ____ ( name of applicant & address)
7 Demand draft for Rs.500 in favour of CTO ___________ circle.
PARTNERSHIP FIRM
1 Identity proof for each partner : Ration card, Voter Id or passport.
2 Passport size photographs of partner : 5 nos for each partner
3 If busines premises is own :Property title deed, property tax challan.
4 If busines premises is rental : copy of rental agreement, electricity bill for 2 months
