Corrections which can be made in tds/tax payment challans

In the link given above there is clarification regarding the corrections that can be made in TDS/tax payment challans. There are some changes that can be made only by the assessing officer for the challans that are dated prior to september 01 of 2011. My queery is, whether ther is any prescribed form by which we have to apply for such correction. If yes, then please mention the form no. or link to the web page where I can find such form.

Sir, your link really helped me, but my actual question was regarding the form in which we should apply for correction of challan with the concerned AO of the assesse. If anybody has format of that would be of great help.
