There is no limit under the Companies Act, 1956 that how many times a company can change their name.

In form 1B it is asked to mention name of the company at the time of incorporation to be shown in fresh incorporation certificate.
But we are changing name of the company for the second time, so i have to give name at the time of incorporation or changed name ??

If the form demand the name at the time of incorporation then you will mentioned that name and not change name.
